Ask for the quote
She had every row of her own journal in front of her and still misreported what it said, until the question changed shape. An open question over the set (what have you written about this?) asks the model to synthesize, and synthesis is where fabrication lives. A closed lookup (quote that entry exactly) asks for a copy, and a copy either matches the record or it doesn’t.
That’s the whole detector: ask for the exact quote and check it. Fabrication cannot survive a verbatim request that gets checked; one string comparison settles it, and the ask costs a single sentence.
The two-act finding behind the diagram, including the morning the record was in her hands and she still got it wrong, is in The verbatim test.
