Same record, two question shapes
Every row was already in front of her: and an open question still came back wrong, while a demand for the exact quote came back byte-perfect. Fabrication lives in the synthesis step; a verbatim demand skips that step entirely.
The same record, questioned two ways. An open question, what have you written about X?, invites her to synthesize over the set, and synthesis is where fabrication lives: she misreported rows that were sitting in her context. A closed lookup, quote that entry exactly, leaves no room to compose, which makes the verbatim demand a cheap, reliable fabrication detector.
