Done is not a sentence: it is an object the code mints only after evidence passes its postconditions. The narration comes after, describing a mint that already happened, and a claim with no artifact has no path to it.

Completion items are minted by code, never asserted in prose: work produces evidence, postconditions check it, and only then does a completion exist for the narration to describe. A claim without an artifact simply has no route to done. And because a file can exist while its contents are invented, the mint also demands that every claim carry a verbatim snippet from something actually read: a lie has nothing to be about.
